Applying For Services ...

Application for services can be obtained at any of the Tribal Health facilities or download the application here. Locations include St. Ignatius, Arlee, Ronan, Polson and Elmo. Submission of the applications must bear the applicant’s original, physical signature.

Individual’s seeking to apply for services can remit their application to any of the clinic locations or send them to:

For updating applicant information the process is similar. However, once you’ve applied, your information is then assigned to a Beneficiary Specialist at the St. Ignatius location. The internal management disbursal of applicant information is on a last name basis as follows:

Should applicants have any questions regarding this process or services available, the Beneficiary Specialist are available Monday through Friday, 8:00 a.m. until 4:30 p.m. to provide the necessary assistance.
